The Clinical Nurse Manager is responsible for daily activities of the health center as it relates to patient care and treatment.
Responsibilities
Works closely with health center administration and managers to ensure health center operates in coordinated, organized manner such that quality care and services are provided.
Serves as a member of Quality Assurance, Infection Control and other committees as deemed necessary.
Participates in development, review, revision, implementation, and monitoring of policies/procedures at the direction of supervisor.
Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.
Supervises LPNs and other medical staff as necessary in all clinical activities.
Care Manager for patients with complex medical and behavioral health needs.
Develops plan of care individualized for the patient reflecting collaboration with other members of the healthcare team.
HEDIS measure tracking and reporting to Quality Assurance Committee and Health Plans.
Prepares Pre-Visit Planning to improve Quality outcomes for the patient and provider.
Participates in daily huddle identifying specific needs for successful patient encounters.
ER/Hospital/Urgent Care Discharge planning, care coordination and transition of care.
Utilize NYSIIS for immunization tracking/upload.
Participates in Chronic Care Management/Population Health Management Programs.
Interviews patients as indicated to gather pertinent medical and social history and medication reconciliation
Assist in recruitment, training and mentoring of nursing and clinical staff.
Perform medication and procedure prior authorizations.
Administer injections as requested by MD/NP.
Consult with supervisors and physicians to determine optimal treatment plans for patients.
Flexibility to work in both health center locations as assigned.
Oversee inventory and ensure ordering of necessary medical supplies and materials.
Ensure health center staff are up to date with FIT testing and assist in testing as necessary.
Oversee/maintain logbooks for quality control.
Maintains current competency in nursing specialty by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
Completes assignments as requested by the supervisor.
May be required to assist with the following as needed:
Prepare examination/treatment rooms; measure vital signs (i.e., pulse rate, temperature, blood pressure). as well as height and weight and records information in Electronic Medical Record (EMR).
Clean and sterilize instruments as needed.
Operate electronic electrocardiograph (EKG) and other diagnostic testing.
Administer treatments and perform routine laboratory tests as needed.
Qualifications
Associate degree in Nursing Required. Bachelor’s Degree in Nursing, a plus.
Current license as Registered Professional Nurse in New York State.
Excellent verbal and written communication skills.
Strong oral, written and organizational skills.
Proficient computer skills.

Strong commitment to the provision of quality medical care for medically underserved individuals.
Proven capacity to work productively in highly collaborative settings and to seek and synthesize input from multiple stakeholders.
Knowledge of Public Health practices/procedures, HRSA regulations, federal regulations and EMR, a plus.
