Overview

The Community Outreach Coordinator is responsible for outreach to the uninsured, under-insured and medically underserved areas and populations as identified in the Advantage Care FQHC Community Needs Study.  Community Outreach Coordinator will identify those in need of services which include, but are not limited to, preventive primary health care services, including care for chronic and communicable diseases, dental services, mental health services, pediatric care and immunizations.

Responsibilities

  • Develop and implement a comprehensive and innovative community outreach plan targeting individuals, corporations, schools, places of worship, and other organizations.
  • Conduct research and collaborate in developing strategic plans, based on a thorough needs assessment, to expand Advantage Care's presence and reach more underserved patients.
  • Organize and host community events, presentations, and workshops.
  • Educate prospective patients about obtaining essential support services such as Medicaid, Exchange, and other managed care health insurance coverage.
  • Facilitate ongoing community and center-based patient registration with participating insurance companies.
  • Connect patients to social care networks to address health-related social needs.
  • Liaise with insurance companies to review panels and promote patient enrollment in the health center.
  • Obtain linkage agreements with community partners.
  • Serve as an ambassador of the mission of Advantage Care as a Federally Qualified Patient Centered Medical Home (PCMH) in all interactions with current and potential partners.
  • Oversee and schedule website updates and strategic/digital marketing/event campaigns to ensure adherence to campaign, event, and calendar deadlines.
  • Collaborate with marketing agencies to support initiatives that assist in the expansion of Advantage Care’s brand and overall presence.
  • Manage online communities to ensure respectful and appropriate engagement.
  • Assist in fundraising efforts and grant applications.
  • Track, document, and report on community outreach and engagement activities.
  • Provide regular updates to management on outreach activities and progress towards goals.
  • Support government and patient advocacy efforts that ensure participation in educational opportunities.
  • Liaison with CHCANYS Advocacy team.
  • Liaise with patient satisfaction vendor.
  • Perform other duties as requested by supervisor.

Qualifications

  • Associate’s degree required; bachelor’s degree preferred.
  • Minimum 1 year of community outreach and marketing experience.
  • Ability to communicate in English and Spanish, both orally and written preferred.
  • Proficiency in Microsoft Office and EMR experience preferred.
  • Reliable transportation.
  • Valid Driver’s License.
  • Basic knowledge of target populations’ ethnic and medical needs.
  • Strong interpersonal and communication skills; demonstrates empathy and cultural sensitivity.
  • Ability to relate to patients and community organization representatives.
  • Works independently and in a multi-disciplinary team with medical and mental health professionals.
  • Experience in case management, data management, and event planning.
  • Proficiency in Microsoft Office; EMR experience preferred.

Physical Requirements

 

  • Mobility: Ability to move around various event locations, including walking, standing, and occasionally lifting materials up to 25 pounds.
  • Travel: Frequent travel to community events, which may include driving or using public transportation.
  • Stamina: Ability to work extended hours, including evenings and weekends, as required by event schedules.
  • Communication: Strong verbal communication skills for interacting with community members and partners in potentially noisy environments.
  • Manual Dexterity: Ability to handle and set up event materials, such as banners, tables, and promotional items.
  • Visual and Auditory: Adequate vision and hearing to interact with participants and monitor event activities effectively.
